Stunts gone wrong that are still in the movies?

My wife and me happened to be watching Big Momma's House 2 (We're on a 'let's watch really bad movies and make fun of them' kick) and there's a scene in the film where Malcolm / Big Momma is chasing Bishop down the boardwalk: https://www.youtube.com/watch?v=YZeKw2-86pw

And the end of this chase it's very clear that the stunt went wrong somehow when the stuntman jumps and lands on the boat. He seems to immediately go limp and then he hits his head, not seeming to land correctly. (In the film, this character dies in this scene because he's been impaled by a inexplicable spike on the boat; but it's never mentioned and the character - who was a seemingly important part of the film - is never mentioned again)

This made me wonder about stunts in films that didn't work correctly that are still in the movie. (If anyone knows about this stunt in BMH2 for certain, please feel free to tell its story!)
